On payment of a fee, the Western Australian Registry of Births, Deaths and Marriages can provide a certificate from a registration of birth, death or marriage that occurred and was registered in Western Australia since 1 September 1841 and a certificate of a change of name document registered at this office since 31 January 1957.  If the event occurred in another State, see Locations for interstate contact details.

Prior to issuing certificates, original identification  documents need to be shown at the registry counter or clear photocopied identification documents must be posted with your application.
