Capture the excitement and joy of a special birth in a unique and lasting way with a Commemorative Birth Certificate. These certificates are available for anyone born in Western Australia, and make the perfect gift to celebrate the birth of a newborn child or preserve family history.

The specially designed birth certificates are printed on archival quality paper and come in a standard framing size of 210mm x 297mm (A4).

  • Baby Prints - a timeless keepsake by personalising your child's birth certificate with their hand or foot prints on the document. Illustrated is a sample of the certificate with an infant's foot prints (maximum 'prints' height 12 cm).

When you purchase a commemorative birth certificate, you also receive a standard birth certificate at no extra cost. This document is for everyday use such as passport application, opening a bank account and personal identification.

How do I apply for a certificate?

To order your commemorative certificate, simply fill in the details on the order form and post it to the registry. Payment of a fee is required with your application.

Identification requirements

Prior to issue of any certificate, original identification documents need to be shown at the registry counter or clear photocopied identification documents must be posted with your application.

Commemorative birth certificate applications for children

A parent named on a child's birth certificate can apply for the certificate. That parent's identification must be provided with the application. 

A person must be at least 16 years of age to apply for their own birth certificate and must provide their identification.

A child under the age of 16 years applying for their own birth certificate must normally have the written consent from a parent named in the certificate and that parent's identification. See Letter of Authority download below.

Certificate applications that relate to a person other than yourself

If the certificate relates to a person other than yourself and you do not qualify under the Registry's Certificate Access Policy, you must provide the written consent or authority from an authorised person and you must also provide identification for yourself and the person for whom you are acting, see Letter of Authority download below.

There is unrestricted access, and no identification requirements are needed for births older than 75 years, deaths older than 25 years and marriages older than 60 years.
